PAAPC Registration Form – July 18, 2024

As a condition of registration and participation in this course, all students will be required to grant permission to the rights of their image, likeness and sound of their voice as recorded on audio or video tape without payment or any other consideration. Students understand that their image may be edited, copied, exhibited, published or distributed and waive the right to inspect or approve the finished product wherein their likeness appears; waive any right to royalties or other compensation arising or related to the use of their image or recording. Students will be required to sign a release which can be reviewed here.

A refund request must be made in writing within 24 hours of your payment ONLY. Full refunds will be issued less an administrative fee of $100.00. Your request MUST include your full legal name and email address. Your request MUST be emailed to support@smcdoulas.com.
