If I notice my client is not producing enough milk and baby was not getting enough milk, I would approach her kindness, compassion and without judgement. I’d bring these observations assuring her that low milk supply is not her fault, she is not alone, and that there are many things we can do and try to help get her supply flowing. First I’d like to know what she feels is the biggest challenge/hurdle because this might reveal something important. For example, maybe she is not in a comfortable position, she might be in pain, maybe she has not been eating well or resting enough. It will be important for me to ask her about things like rest/sleep, mental state, eating habits, hydration and any other factors that could be impacting her milk supply.

    2. I would consider showing mom some visual about comfortable feeding positions/latching techniques while reassure her that breastfeeding takes practice and continuing breastfeeding will trigger milk supply to come in. I would connect mom with a local lactation consultant or one she may have already been connected to at the hospital and encourage for an in-person visit as in-person support might be very helpful. I’d share with her the Breast Friends hotline so that she can speak to a lactation consultant at any point.

    3. As the doula, my first priority would be to emotionally support my client, encouraging and empowering them to believe in their natural ability to feed their baby and get her connected to resources that can support her and baby. I would also encourage the father/partner to support mom during breastfeeding by taking an active role in this process, if not already.

    4. I would absolutely encourage the mom to reach out to her pediatrician if there are concerns around the baby not getting enough milk. As the doula, it will be important for me to direct my client to her medical providers and stay within my scope of work of providing emotional support and encouragement to ensure baby and mom are both healthy.

Great post you covered every detail. I would be very happy with you as my doula in this situation.
